 - Must be fully vaccinated

Shoot Date: July 5-7, 2022
Shoot Location: Chicagoland Area, Illinois
Rate: $120/8 minus Agency Fee + $60/4 COVID Test

6) List your current sizes: Jacket, chest/dress, shirt (neck/sleeve), pants waist, head size and shoes. Please list your height and weight as well, and update these in POP if they are not currently accurate or listed.

7) Indicate your mode of transportation, i.e. Uber, Bus, Own Vehicle, etc.

8 ) Please confirm you are not experiencing any Covid symptoms and that you have no known exposure to Covid.

9) Confirm you are willing to take a Covid test provided by the production.
